		<description>that&#039;s good but it&#039;s hard to devise a real system for EHRs with out a real health care system.  I do think the first step should be to establish so-called &quot;medical homes&quot; for each person- typically this would be a large medical center, university or otherwise, where primary care providers (doc and nurses) would have formal relationships.  the primary care provider and the patient would be responsible to be certain the EHR was accurate and up to date.  the data would be accessed through the medical home&#039;s data base just like many of us already do with our local banks, our Vangard funds, etc- with a password, one can assess his own data.  The next step, of course, is to find a way to link the data from one patient with all other medical homes.  Actually, the only technological difficulty is security.  Noen of this stuff is very hard to do,  I doubt it will save much money but it will GREATLY improve patient care.</description>
